Chronic Care Management – An Overview

In 2015 Medicare began reimbursing practices for the non-face-to-face care they provide, a breakthrough in how healthcare services are compensated. Medicare recognized that medical practices are spending a great deal of time and money managing patients with multiple chronic diseases while they were not in the office. The Chronic Care Management services program now pays approximately $500 a year per patient when these services are performed monthly. The primary requirement is that clinical staff members spend at least 20 minutes per month in non-face-to-face services.
